We offer a variety of rolling group therapy sessions through our Black Visions of Wellness program as well as for Chronic Diseases and Diabetes.

Whether it’s housing insecurity, food insecurity, or something else that is impacting your life, our bilingual case managers are available to connect you with the resources you need and help you navigate the path to a healthier life.

If you’re an UMMA patient living with a chronic disease or condition, we can help.

Learn how to live better with a chronic disease or illness by signing up for our 6-week chronic disease self-management program. There will be a two and a half hour class each week for six weeks led by two trained leaders.

Subjects covered include:

  • Techniques to deal with problems such as frustration, fatigue, pain and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends and health professionals
  • Nutrition
  • Decision making
  • How to evaluate new treatments

We currently offer two individual diabetes education and counseling sessions to all UMMA patients. All sessions require an appointment and patients primarily participate by provider referral.

Any UMMA patient can request an appointment by calling David Reynoso at 323-406-5771.

  • Session 1 – Covers diabetes basics, HgA1c, dietary guidelines, physical activity, complications, and action planning.
  • Session 2 – Provides an overview of healthy eating, MyPlate, and carbohydrate counting.
