Under the supervision of the Medical Director and Director of Clinical Operations, the Licensed Vocation Nurse works within a multi-disciplinary health care team in the delivery of comprehensive services for adult patients diagnosed with chronic illness and high risk pediatric patients in a primary care setting. Acting as the primary contact person for the family, the LVN assists the health care team in the development and implementation of a health care plan tailored to the needs of the client and the client’s families in order to promote continuity of care, improve referral tracking and ultimately reduce the rate and severity of chronic illness complications through the health care team approach.
The LVN assists the physician in the provision of care that is accessible, continuous, comprehensive, evidence-based, patient centered, coordinated, compassionate, and culturally effective. The LVN assists with clinical competencies and provides support and training to the clinical health center staff. The LVN provides technical nursing duties, including but not limited to medication dispensing, ordering, stocking and dealing with expired medications in the dispensary. The LVN coordinates with health center staff to provide patient education. The LVN’s role is to ensure safety by maintaining the emergency kits and providing staff orientation and trainings on medical emergency equipment and procedures. The LVN performs work under the supervision of a licensed physician.
The Licensed Vocational Nurse is also responsible for providing leadership to clinic Medical Assistants to ensure efficient clinic flow, excellent patient care and customer service. The LVN will perform various clerical duties related to the provision of care. As a leader, he/she will set an exemplary of peer leadership and model consistent back office teamwork.

|Provides technical nursing duties with medication inventory for expirations, ordering and stocking.|
|Ensures safety in the health center by inspecting the emergency kit(s) every month and assisting with medical emergencies. Checks for any expired medications or supplies and orders replacement supplies in a timely and efficient manner. Provides health center training and education to staff on medical emergencies and equipment.|
|Insures all dispensary logs are documented accurately and completes them in a timely and efficient manner: oxygen tank, hot water, cleaning, expired medication, eye wash, room temperature, and refrigerator.|
|Administers injections and provides phlebotomy assistance to lab staff when needed.Participates in patient follow-up activities as directed by providers for abnormal laboratory, DI or specialty consult results. Interacts with patient by phone, text, letter or email/portal.Dispense medications according to approved standardized procedures, including DOT (direct observation therapy), placing TB tests, and giving injections.Performs, under the direction of the DON or Clinic RN, delivery of technical nursing services in caring for acute or chronically ill patients, to including:Assisting provider with examinations or special procedures.Administering medications orally, topically, and by injection.providing nursing services, including routine screening services such as VS, EKG, hearing and vision assessmentsDocument all patient interactions as deemed appropriate in the patient chart in the EMR, taking care to chart in an informative and descriptive manner that reflects the patient care provided and keeping accurate records on actions and patient education.|
|Participates in quality and utilization activities that include the tracking, completion and submission of reports in the dispensary department.|
|Adheres to medication safety practices including data entry, labels and verification of orders. Ensures required dispensary documents are filed in a timely and efficient manner: prior authorizations, pharmacist consultant letters, and in-house medications.|
Participates as part of healthcare team that works to sustain attainment of UMMA certifications including but not limited to the Patient Centered Medical Home and other certifications as required.
Contributes to data collection to measure quality and performance.
Adheres to all company safety policy and procedures. Maintains a safe, clean and organized working environment.
|Provides education to patient/families in a culturally sensitive, appropriate manner.|
|Practices good financial stewardship by minimizing expenses with appropriate medication inventory control and reduction of wasted medications including vaccines.Ensures all vaccine logs, inventories are up to date, ordered and disposed of appropriately and in a timely manner.|

The environment for this position is medical clinic that is clean and comfortable. It may include some minor annoyances such as noise, odors, drafts, etc. The incumbent is in a non-confined office-type setting in which he or she is free to move about at will. In addition, the incumbent may travel via personal vehicle or public transportation throughout the community.

To perform effectively in this position, the incumbent must have:
- 1 year of experience in a community based clinic or medical office environment preferred
- Must hold a current Licensed Vocational Nurse License in the State of California
- Current Provider BLS CPR card
- Bilingual in English and Spanish required, with excellent written and oral communication skills
- Articulate clearly with patients over the phone or in person
- Work effectively with medical assistants, physicians and clinic staff within a complex medical center
- Accept delegation of tasks and presents a positive role model
- Possess the ability to work independently and as a team member
- Possess excellent organizational and problem solving skills and have the ability to prioritize
- Experience with basic computer skills and must type 35 wpm
- Annual physical
- Two professional references from current/former supervisors
- Citizen of the United States or proof of the right to work in the U.S.
Typically, these skills are the result of a combination of education and experience equivalent to a bachelor’s degree that included courses in business, public health, English, accounting, computer skills, supervision and similar, and several years of progressively responsible experience in a medical/clinic office or similar.
